If the sentence you are connecting to another sentence using fanboys has a dependent phrase in front, Is it correct to say:

“My dog went to the gym with me, and, without thinking twice, the manager kicked us out.”
(, and,)
or

“My dog went to the gym with me and, without thinking twice, the manager kicked us out.”
(, and)

I’d use the punctuation in the 2nd sentence. Or even ...

“My dog went to the gym with me, and without thinking twice, the manager kicked us out.”
